How do I know if my doctor is in the network?

  • For Blue Cross Blue Shield group & individual visit: www.bcbsil.com click on Provider Finder.
  • For Aetna group visit: www.aetna.com click on Find a Doctor under shortcuts.
  • For Aetna individual visit: www.aetna.com click on Individual under shortcuts then click on Find a Doctor under shortcuts
  • For Humana One individual & Humana group visit: www.humana.com and click on providers then choose from the Provider Search list.

What can I do about my premium increase?
Please call your agent to discuss other options within the same company or a different company.

I don’t think that my claims were paid properly. What should I do?
Collect all of your EOB’s (claim adjustments) and billing statements. Call your agent immediately to review claims and file appeals.

When do I call to add a newborn to my policy?
You must add a newborn to your policy within the first 30 days of birth.

How do I know if I have the right Medicare Part D prescription benefit?
Go to www.medicare.gov and enter your prescriptions, dosages, and desired pharmacy for a complete search of the lowest cost program available.

How do I change my address?
